Participants must pick up their paperwork at the City of Altus Recreation Department for their physical.
If the participants choose to get a physical at the doctor of their choice, they must have the physical information forms completed with the City of Altus.
If participants cannot afford to pay for their physical or need more information regarding the Altus football program they may contact Amy or Ulysees at 481-2265 or 481-2266.
